Step 1:
Be able to prove your 12 months sober:
Most AA programs will be able to prove this 12 month sobriety.
12-month MRT will proof will be accepted as well
Step 2:
1) Collect your 12 month driving record from the secretary of state link
2)This demographic form is needed in advance of your assessment. Your appointment will not be scheduled until payment and completed documents are received.
